Description:
1. The subject of the order is the successive delivery of medicines used as part of drug programs, chemotherapy, health services contracted separately and supportive medicines for the needs of the Provincial Hospital. Dr. Roman Ostrzycki in Konin.
2.The subject of the order has been described in detail in Annex No. 2 to the SWZ (Assortment and Price Form), which provides the quantity of the subject of the order and the description of the subject of the order for the basic order and for the order covered by the option right.
Item the order was divided into 162 parts (packages).
4. The Ordering Party allows the submission of an offer for one, several or all parts.
5. The subject of the order will be delivered at the expense, risk and transport of the Contractor.
6. The conditions for the implementation of the Subject of the Order are also included in the designed ones provisions of the contract, constituting Annex No. 5 to the SWZ.
7. The minimum guaranteed level of the value of orders that the Ordering Party undertakes to place (minimum guaranteed level of use of the contract value) is specified in the Draft Provisions of the Agreement constituting Annex No. 5 to SWZ.
8. The Ordering Party does not allow the submission of variant offers.
9. The Ordering Party allows the possibility of exercising the option right.
